You will need to create a unique User ID, if the name you select is already being used,
the system will automatically add a number to it so that it will have a 1, 2, etc. at the end of it (e.g.,
John1). The number that is created will appear at the top of the screen when you
enter the survey. Please make a note of your name and number. If you need to return to the
survey at a later time, enter this User ID and number on the main logon screen.
